Fourth Avenue Homes Personal Care Assistance

Through Fourth Avenue Homes personal care assistance program, we can provide comprehensive services tailored to each person's needs — giving them a sense of independence while still allowing them access to quality resources. Our goal is to help qualified individuals maintain their independence while living in their home.

What are Personal Care Attendant Services?

Minnesota strives to allow individuals living with a disability the opportunity to remain in their homes rather than relocating into institutional care. Established almost four decades ago, the state's Personal Care Assistance (PCA) Program assists residents by providing non-medical health support and daily activities essential for independent living within their community. PCA Services ensure citizens of Minnesota can enjoy life while remaining safe amidst familiar surroundings.

How Can the PCA Staff Help with Independence and Care?

PCAs provide essential and invaluable support to disabled individuals, from helping with basic daily needs such as getting dressed or brushing teeth to assisting with more complex medical tasks. They also actively observe and redirect behaviors to ensure that those under their care are physically and emotionally safe.

Enjoying autonomy and the security of care within their home is what you can expect when choosing Fourth Avenue Homes. Our trained healthcare professionals are devoted to helping you live life on your own terms. Through a personalized plan tailored to each individual's needs and goals, we strive to provide the necessary care and support that enables people to lead independent lives full of purpose and fulfillment.

Personal Care Assistance Services Eligibility

In order to access PCA services, individuals must be living in the community and not in a facility like a hospital or nursing home. Furthermore, they should either have their own care representatives or someone who can direct it on their behalf; likewise, being enrolled into one of Minnesota’s health care programs (Medical Assistance/MA waiver) is required — while some people under Minnesota Care may also deem eligible. An assessment will decide if an individual meets all criteria for enrollment. Discussions with county tribal nations/managed organizations are necessary at this stage.
