We Support Each Person

Our team has the know-how to help adapt a home to meet a person’s specific living needs in hopes of accelerating both their independence and comfort.

What We Do:

  • Teach essential living skills to build independence
  • Demonstrate and help with regular household duties
  • Further their connection with their local community and other community organizations through socialization and participation opportunities
  • Develop health and wellness habits and awareness
  • Assist with various activities central to daily living
  • Provide transport to and participate in community inclusive activities
  • Conduct medication oversight and scheduling
  • Provide advice on money management and budgeting
  • Help with behavioral support

Disabilities Services

Fourth Avenue Homes’ services are perfect for people who require case-based assistance but not highly structured support as they want to grow their independence.

The individuals we support include:

  • people with disabilities who need scheduled care assistance, and advocacy
  • people with mental health issues (including PTSD)
  • people suffering from a brain injury

Waivered Programs

A person may qualify for our services via the Waiver Program through the Department of Human Services of Minnesota (MNDHS).
