Nursing Home Level of Care in a Residential Setting
Fourth Avenue Homes allows older adults age 65 or older to live independently, while also receiving an appropriate level of care and support. We firmly believe that when older adults are valued like this, they are more comfortable and likely to prosper. That’s why our staff is available to them 24 hours a day, 7 days a week, every day of the year!

Fourth Avenue Homes' services are ideal for seniors who want case-based assistance but not the institutional environment of a nursing home facility.
Elderly Waiver Program
An elderly person may qualify for our services via the Elderly Waiver Program through the Department of Human Services of Minnesota (MNDHS).

The eligibility requirements for this waiver are:
- Must be aged over 65 years old
- Qualify for Medical Assistance
- Require a nursing home level of care as recommended after a Long-Term Care Consultation
- Require services that the Elderly Waiver can provide for less than the cost of care in a nursing home facility
