What We Do:
Disabilities Services
Fourth Avenue Homes’ services are perfect for people who require case-based assistance but not highly structured support as they want to grow their independence.
The individuals we support include:
- people with disabilities who need scheduled care assistance, and advocacy
- people with mental health issues (including PTSD)
- people suffering from a brain injury
Waivered Programs
A person may qualify for our services via the Waiver Program through the Department of Human Services of Minnesota (MNDHS).
