Everyone should feel valued and encouraged to realize their potential, and we aim to provide Community Alternative Care services for people to achieve these even when facing the most difficult personal health situations.
Providing comprehensive Community Alternative Care Services
Fourth Avenue Homes is passionate about helping people suffering from chronic illnesses in a residential setting instead of in a medical facility. When people are in need of highly structured Community Alternative Care and are comfortable with those around them, they are more likely to thrive!
We provide:
24/7 Hospital-style Support
Individualized or case-based care
In a residential setting!
When someone is chronically ill or suffering from a debilitating condition, it often leaves them feeling vulnerable and isolated. It may even seem like the whole world is leaving them behind, and there is no end in sight.
Services We Provide
At Fourth Avenue Homes, we tailor our support to the person. With our help, they will:
Serving Chronically Ill Individuals
Our services are ideal for chronically ill people who need case-based help and want to maintain some of their independence.
When someone is facing chronic illness or is medically fragile, they typically need round-the-clock care that family and friends may not be able to provide.
Hospitals and other respite care solutions aren’t always the answer either.
But that’s where Fourth Avenue Homes steps in!
We provide the 24/7 Hospital-styled support these individuals need, all in a home-based setting.
Community Alternative Care Waiver
A person may qualify for our services via the Federally approved CAC (Community Alternative Care) waiver plan in the state of Minnesota. These waivers cover home care services for a person when they require a hospital standard level of care. This waiver will help a person live as independently as possible in a community setting to promote better health, further their independence and safety, as well as integrate them more in their community.
Visit the Centers for Medicare & Medicaid Services (CMS) website at medicaid.gov now to see if you or someone else meets the broad criteria for our services that may come at a minimal (or zero) cost to you.
